Anthony McCoy è un pittore in crisi creativa, stimolato dalla compagna Brianna, che lavora nel campo a livello organizzativo, a darsi da fare. Anthony trova improvvisa ispirazione in modo inaspettato. Troy, fratello chiacchierone di Brianna, viene infatti una sera in visita con il suo compagno Grady e racconta la truce, ma vera, vicenda di Helen, una studiosa che anni prima era venuta a compiere delle ricerche nel quartiere ed era apparentemente impazzita, uccidendo un rottweiler e rapendo un bambino prima di bruciare in un rogo. Anthony scopre, grazie anche all'incontro con il titolare di una lavanderia a gettone, che dietro a tutto c'è la leggenda di Candyman: se tu pronunci cinque volte il suo nome davanti allo specchio lui arriverà per ucciderti. Anthony dipinge a più non posso, ma qualcosa di terribile inizia ad accadere intorno a lui.
